What a colossal failure of fiction when it turns out most of the plot points came from different committees not communicating with each other.
When they were making "It's a Wonderful Life," Uncle Billy walks off-stage, inebriated, three sheets to the wind. Off-camera, a grip makes a mistake a drops some equipment, which makes a horrible racket. The actor playing Billy says, "It's okay, I'm all right." Capra gives the grip a bonus for his accident, and that scene - totally by accident - was so good they used it in the movie.

They also found out during filming about the swimming pool beneath the gymnasium at Beverly Hills High School so it got written into the script.

Sometimes mistakes do work out okay.

But there is another issue. Star Trek:TOS did much better on some of the stuff they did because they were under severe resource conditions. The teleporter - which is a brilliant gimmick - was invented because they couldn't afford to do a filming of the ship actually landing on a planet. McCoy's medical instruments were actually intended to be 24th Century salt and pepper shakers for a dinner table used in a scene until someone just decided to boost a set from the commissary, use those, and use the high-tech salt shaker as a medical instrument. In a lot of cases, something got done because when they needed a prop, the guy quoted a price it would cost, perhaps $3,000, and the prop department manager tells him they have $568 in the budget, that's all they can afford, the guy can't figure out how he can do it, but then he comes up with something creative and figures out that, yeah, he can do it for $568.

I think the subsequent Star Trek shows and the related movies got spoiled because they could spend like drunken sailors at a whorehouse, so they weren't forced to be creative and innovate, to stretch and skimp and get by with scrounged materials and do what you can with what you have. Too much money can cause problems. Among them, a lack of real creativity by not having to find solutions that would work, on low resources.
