Does it make significant changes?

Is it possible to just order a CD and install it, or must I have high speed internet access and an Xbox live account for it?

None of the changes work on old savegames. You've got to start a new game to take advantage of the Hurricane Pack. Depending on how deep you are into the game, it's probably not worth it.

The camera can now be controlled manually. The biggest improvement, and the most welcome.

A lot of the enemies are tougher, because the one complaint everyone had about Ninja Gaiden was that it was too fucking easy.

Xbox Live is the only way to get the update. Team Ninja has hinted at future updates as well.
