"Carrot and stick" refers to a system of reward and punishment used to teach you how to play a game. If you learn to dodge his punch, then counter-punch, you get the carrot (a new challenger); if you don't, you get the stick (sent back to Glass Joe).

"Carrot on a stick" refers to the empty promise of reward hanging perpetually just out of reach, used to keep you doing the same thing over and over again until you either wise up or die of exhaustion. I don't think Bethesda is unaware of the difference.
