According to my calculations, Fallout 3 is the eighth-greatest game ever made. It is unfortunate that the 8th greatest work in a field that has existed for roughly 30 years has such intolerable plot holes, as the ones you've discussed. But whatever, shooting people in VATS is more fun than watching the 8th best movie or listening to the 8th best song.
I would invite you to continue your analysis of the game, as I genuinely find it compelling.
